Add CSFP Closure Notification Letter

This form is used to notify a participant that they are no longer eligible for the CSFP program. Once this letter is provided to the participant, they have 60 days to appeal the decision.

From the neighbor’s case, select the “CSFP” tab. If the CSFP tab is not visible, contact your CSFP or Oasis lead to have it added. Select “Add Form.” From the drop-down menu, choose “CSFP Closure Notification Letter” and then select “Add CSFP Form.”

Complete all fields and then select “Save & Close.” To view the completed form, select “Print Closure Notification Letter.”
