This is issued to participants, who then have 15 days to respond to the notice.

This form is used to formally notify a participant that they are being terminated from the program. This notice must be provided to the participant at least 15 days before the effective date of discontinuance and include the reason for termination.

From the CSFP tab, select “Add Form.”

From the drop-down menu, choose “CSFP Disqualification Notice” and then select “Add CSFP Form.”

Complete all fields and then select “Save & Close.”
