Add the CSFP Recertification
This form should be completed once a year for each participant. It is important to understand if a participant needs a recertification or an extension. This is determined by the type of income they receive. If someone has fixed income, they will get an extension every year then a recertification on the third year.
Participants with variable income get a recertification every year. For a recertification, participants should provide their documents for review, i.e., driver’s license, proof of income, etc.
From the neighbor’s case, select the “CSFP” tab. If the CSFP tab is not visible, contact your CSFP or Oasis lead to have it added. Select “Add Form.” From the drop-down menu, choose “CSFP Recertification/Extension” and “Add CSFP Form.”
Choose the appropriate income type, then select “Recertification.” A recertification will populate additional fields since it is a longer form, so please be sure to complete all applicable fields. When finished, select “Save & Close.” To view the completed form, select “Print CSFP Recertification/Extension Form.”

Choose the appropriate income type, then select “Extension.” When finished, select “Save & Close.” To view the completed form, select “Print CSFP Recertification/Extension Form.”
