Add the CSFP Verification of Certification Form

This form is used for if a participant moves to a new location during their current certification period. The form is used to prove to a new CSFP site that the participant is already certified and can be accepted as a participant there. The neighbor/participant can request the form from their current CSFP site.

From the neighbor’s case, select the “CSFP” tab. If the CSFP tab is not visible, contact your CSFP or Oasis lead to have it added. Select “Add Form.” From the drop-down menu, choose “CSFP Verification of Certification Form” and “Add CSFP Form.”

Complete all fields and then select “Save & Close.” To view the completed form, select “Print CSFP Verification of Certification Form.”
